ABOUT THE FORMS

These are the official forms from the national office of CFGPI, available to Foursquare churches nationwide and may be downloaded and printed for official use only. Each item of the form has corresponding password which may be requested from your district office. To download, select and click an item from the list and a popup box for password will appear.

The respective district office may provide the requested password for the specific item only and or for other downloadable and password-protected materials upon request. The district is not allowed to share all the passwords all at once. It is a "per request per item basis," aside from the fact that these passwords shall be changed every three months for security and control.
